Mobility Device Assessment & Access to Funding
Supporting Safe Movement and Fall Prevention at Home and in the Community
As we age or following an illness or injury, we may have difficulty standing or walking. We assess how well a person moves—strength, balance, coordination, and gait—to identify risks that may lead to falls or limit independence.
Whether you’re concerned about walking, transfers, or using mobility aids, we create a customized plan that promotes safety and confidence.
What We Assess:
- Muscle strength, range of motion and balance
- Cognitive and neurological factors impacting mobility (e.g., dementia, stroke)
- Caregiver support needs
- Environmental variables
- Use of assistive devices (walkers, rollators, wheelchairs)
What You’ll Receive:
- Personalized recommendations for mobility aids
- Referrals to trusted vendors and support in ordering the right equipment
- As an authorized provider under Ontario’s Assistive Devices Program, we help eligible individuals access financial support—covering up to 75% of the cost of primary use mobility devices.
- Education on safe movement and fall recovery
- Guidance on improving functional mobility and daily routines
